Text, call, email or video
Many questions don’t need a trip in. Members reach us by text, phone, email, or video at no additional fee.
Handled in the office
EKGs, rapid strep tests, urinalysis, stitches, joint aspiration, and ear-wax or benign skin-lesion removal.
Annual wellness exam
A personalized, in-depth wellness exam each year, with a custom wellness plan.
Discounted labs & imaging
Quick tests like a strep test or urine dip are done in the office at no charge. Bloodwork such as an A1C or lipid panel, and imaging, are offered at discounted rates, or your company can choose to cover them.

Services as described in the Employer Agreement (Appendix A), as deemed appropriate and medically necessary by the physician. Some services carry an additional fee, including send-off labs, imaging, and vaccinations; your agreement lets you choose whether your business covers them. Chronic disease management excludes pain management. Specialist fees are not included. Urgent sick access is never for emergencies. In an emergency, call 911.

Book a 15-minute callIs this health insurance?
No. Paradigm is a direct primary care membership, not health insurance, and it isn’t a substitute for any health plan you offer. It pairs with any plan, including a high-deductible health plan (HDHP).
We recommend talking with a health insurance professional about coverage that complements direct primary care. Members can still use their insurance for things like referrals, labs, vaccines, and imaging.
What costs extra?
Membership includes office and telemedicine visits. Some services carry an additional fee, such as send-off labs, imaging, vaccinations, and certain injections.
In your agreement you choose whether your business covers those extras, covers some of them, or leaves them to your employees.
How is billing handled?
You receive one invoice at the start of each month through your employer portal, covering everyone on your roster. Pay by ACH bank draft or credit card. You keep the roster current in the same portal.
Can employees add family members?
Yes, at your discretion. Family members you authorize are added to your roster alongside employees and billed at the adult (18+) or child (0–17) rate.
Is my team big enough?
Employer pricing starts at 2 enrolled adults: one rate for 2 to 4 adults, and a lower rate once 5 or more adults enroll. Enrolling just one adult? Our private membership is the right fit. Not sure where you land? Book a 15-minute call.
What’s the commitment?
The agreement runs for one year and renews automatically. Either side can end it without cause with 30 days’ written notice.
Where is the clinic?
6301 Gaston Avenue, Suite 725, Dallas, TX 75214, in Lakewood Tower (the tall building with the Wells Fargo sign) in East Dallas. Visits are by appointment, so time is set aside for each member.
